Sandra Perrotta, Salve Regina University

Abstract

It is evident that there are numerous individuals living with chronic/serious illness residing in skilled nursing facilities. Identification of these patients would be essential in improving quality care to these individuals in this setting. One purpose of this paper is to review the existing literature of palliative care barriers, interdisciplinary knowledge of palliative care and the implementation of a validated tool in the identification of seriously ill patients living in a skilled nursing facility. An additional purpose is to attain validation that further evidence is needed for implementation of a validated tool to identify palliative appropriate individuals in the setting of a skilled nursing facility.
